1Question

Tell me about a time you supported a service user through a significant step towards greater independence.

Situation: I was key worker for a 34-year-old service user with anxiety and depression who had been in supported accommodation for 18 months and was ready to move on. Task: My role was to prepare her for independent tenancy within a 3-month transition plan. Action: I worked with her weekly to build budgeting skills, accompanied her to 2 housing viewings, liaised with the local authority housing team, and created a graduated support reduction schedule. I also connected her with a community mental health group to maintain her social network post-move. Result: She successfully moved into her own flat on schedule, and at her 6-week review reported feeling confident managing her tenancy independently. Her support hours were reduced from 10 to 2 per week.
2Question

Give an example of when you identified a safeguarding concern and explain the steps you took.

Situation: During a home visit, I noticed a service user with a history of self-harm appeared withdrawn, had unexplained bruising on her arm, and became evasive when I asked about her weekend. Task: I needed to assess the situation carefully without alarming her and follow the correct safeguarding procedure. Action: I calmly extended the visit, used open questions to create space for her to share, and documented my observations in detail immediately after leaving. I raised a safeguarding alert with my line manager the same day and completed a formal referral to the local authority safeguarding team within 2 hours. Result: A safeguarding investigation was opened within 48 hours. The service user received additional support, and the concern was substantiated, leading to a revised safety plan.
