Dear Hiring Manager,

Turning Point's reputation for delivering person-centred drug and alcohol recovery services in Slough is exactly the clinical environment I have been seeking. As an NMC-registered independent prescriber with hands-on experience in opiate substitute treatment and alcohol detoxification, I am well placed to contribute to the prescribing team from day one.

My background in community drug and alcohol services has given me extensive experience conducting detoxification assessments, managing benzodiazepine reductions and administering Pabrinex under clinical supervision. I have worked closely with mental health and social care colleagues to develop flexible, recovery-focused support packages for clients with complex presentations, and I attend regular prescribers' meetings to maintain and develop my clinical practice.

1Question

Tell me about a time you managed a client in crisis during a detoxification process — what did you do and what was the outcome?

Situation: A 38-year-old client on a community alcohol detox protocol presented at day three with a CIWA-Ar score of 18, indicating severe withdrawal risk. Task: I needed to reassess his medication regime immediately and decide whether to escalate to inpatient care. Action: I conducted an urgent clinical review, increased his chlordiazepoxide dose in line with the detox protocol, administered Pabrinex, and contacted the local acute liaison team. I also arranged a same-day welfare check from our social care partner. Result: The client stabilised within four hours and completed his detox safely in the community, avoiding hospital admission. He remained engaged with our OST programme for the following six months.
2Question

Describe a situation where you had to collaborate with mental health or social care services to support a client's recovery. How did you manage differing professional perspectives?

Situation: A client with co-occurring heroin dependency and severe depression was receiving conflicting advice from our prescribing team and the community mental health team regarding buprenorphine dosing and antidepressant initiation. Task: I needed to broker a shared care plan that both teams could commit to. Action: I arranged a joint case conference with the client's care coordinator and consultant psychiatrist, presented the prescribing rationale using NICE guidance, and proposed a staged medication introduction with fortnightly review points. Result: Both teams agreed a unified plan within two weeks. The client's PHQ-9 score dropped from 19 to 11 over eight weeks, and he maintained consistent OST attendance throughout.
