1Question

Tell me about a time you had to make a difficult clinical decision

During a night shift, I noticed an 82-year-old resident with dementia showing signs of confusion and agitation beyond their baseline. Their vital signs were slightly elevated, and they were refusing medication. I assessed the situation carefully, considering their care plan and recent changes. I made the decision to contact the on-call GP rather than wait until morning, as I suspected a possible UTI. The GP agreed to prescribe antibiotics, and within 48 hours the resident's condition improved significantly. My early intervention prevented a potential hospital admission and ensured the resident received appropriate care in familiar surroundings.
2Question

Describe a situation where you had to support a distressed family member

A daughter was extremely upset when her 78-year-old mother with advanced dementia no longer recognised her during visits. She felt guilty about the care home placement and questioned whether her mother was happy. I sat with her privately and explained how dementia affects recognition whilst emphasising that her mother still responded positively to her presence through smiles and calm behaviour. I arranged for her to participate in a care plan review and suggested shorter, more frequent visits during her mother's best times of day. Over 3 months, the daughter became more confident and developed a new way of connecting with her mother through music and photo albums.
