1Question

Tell me about a time you supported someone through a significant personal crisis — what was your approach and what was the outcome?

Situation: A 34-year-old client on my caseload disclosed a relapse following a family bereavement, presenting with increased risk and disengagement from their care plan. Task: I needed to re-engage them quickly and safely while managing an elevated safeguarding risk. Action: I arranged an unplanned home visit within 24 hours, conducted a revised risk assessment, and co-produced an updated care plan that incorporated grief support referrals alongside their existing prescribing intervention. I liaised with the community mental health team to arrange a joint review within one week. Result: The client re-engaged fully within a fortnight, attended 8 consecutive weekly key-working sessions, and reported feeling more stable. The safeguarding concern was formally closed at the 6-week review.
2Question

Tell me about a time you worked in partnership with another organisation or professional to achieve a better outcome for a service user.

Situation: A client with dual diagnosis — alcohol dependency and severe anxiety — was repeatedly discharging themselves from hospital before completing detox, creating a cycle of crisis admissions. Task: As their key worker, I needed to coordinate a more joined-up response across health and housing services. Action: I convened a multi-agency meeting with the RDaSH community mental health nurse, the client's GP, and a housing support officer. Together we agreed a shared care protocol, with staggered goals and a named contact at each agency. I updated the care plan to reflect each partner's responsibilities and held fortnightly review calls. Result: The client completed a full community detox for the first time in three years and secured stable housing within four months, with zero unplanned hospital admissions over the following six months.
